DR. MACDOUGALL DOES NOT ACCEPT ANY PATIENTS THAT HAVE: A) BEEN IN A MOTOR VEHICLE ACCIDENT OR B) HAVE ACTIVE WORKMAN'S COMP CASES (NO EXCEPTIONS!!)
Your diagnostic studies should not be more than 12 months old as your anatomy can change. Dr. MacDougall needs the most accurate information available to make an accurate diagnosis.
Prior to us being able to schedule a new patient appointment we require the following items:

After your appointment has been scheduled the new patient paperwork will be sent to your mailing address. Please complete them prior to your appointment. In the event you do not receive the paper work in the mail, the electronic forms are available for download by clicking the "New Patient Packet" button at the bottom on this page.
On the day of your new patient appointment, you MUST bring with you the completed new patient paperwork and the CD containing the MRI/CT imaging. You will need to obtain the CD from the facility where the studies were performed.

- Recent progress notes from your referring physician
- Demographic information
- Insurance referral IF required
